We are hiring a Family Nurse Practitioner to provide medical management as part of a multidisciplinary team with our flagship Trauma and Addiction Program here in the Phoenix, AZ!
This is a 3-12's Friday - Sunday position leaving lots of off time for adventures!

Job Summary:
The Nurse Practitioner provides quality medical assessment, treatment and referral services to all assigned patients. Serves as a functioning member of the multi-disciplinary treatment team.
Responsibilities:
- Provides primary care medical services for assigned patients including, but not limited to History & Physical, medication management, laboratory interpretations, EKG interpretation, continuous care and admission / discharge orders.
- Completes treatment and documents in a timely manner per company policy.
- Active member of the multi-disciplinary treatment team and communicates any medical problems to treatment team.
- Provides on-call services as assigned responding to calls within 15 minutes and if needed reporting to facility promptly.
- Participates in the performance improvement process as requested. Participates in State and Joint Commission surveys as requested.
Requirements:
- NP or PA License in the State of AZ
- 2-3 years of Medical management experience
- Prior Addiction experience preferred.
