Requirements to apply

• Graduate of an accredited school of nursing.
• RN CA State License.
• Certification as a Nurse Practitioner with prescriptive authority
• Current Basic Life Support (BLS) training.
• Current driver's license and proof of auto insurance.
• Minimum of 1 year work experience as a NP doing adult chronic disease management and functioning in PCP role with or without MD oversight. Specific work experience with the frail, elderly population is preferred.
• Current knowledge of chronic care/geriatric issues and best practices, experience with deprescribing and palliative care would be a bonus.

Job Overview

Under supervision of the Medical Director and in collaboration with Physicians and Community-Based Physicians provides assessment, planning, primary care, and continuous medical coverage to participants in an interdisciplinary team environment and in accordance with our clinic’s standards.
