- Facility #1 – School Health Center
- Facility #2 – Community Health Center (Main Clinic)
- School Based Health Schedule is Monday and Wednesday's from 8 AM to 12 PM - then will return to main clinic in the PM from 1 PM to 5 PM
- We are looking for an NP - to help cover School Based Health Center as well.
- Provider will work at main clinic on as follows
- Monday - 1 PM to 5 PM (8 A. M to 12 PM @ SBHC)
- Tuesday- 8 AM to 12 noon - 1 to 5PM (Closed from 12-1)
- Wednesday1 PM to 5 PM (8 A. M to 12 PM @ SBHC)
- Thursday 8 AM to 12 noon - 1 to 5PM (Closed from 12-1)
- Friday 8 AM to 12 noon - 1 to 5PM (Closed from 12-1)
- Schedule is set up for 20-minute visits. Up to 23 patients per day- with consideration that no show rate
- is high, so provider may average about 16 per day.
- Nurse Practitioner will be assigned a Certified Medical Assistant and Customer Access Rep.
- Active NM License
- BLS, PALS, DO
