NURSE PRACTITIONER

Essential Functions

  • Performs health assessments and preventative health measures within prescribed guidelines, protocols, and Extensivist Physician instructions
  • Orders, interprets, and evaluates diagnostic tests to identify and assess * Patient clinical problems and health care needs
  • Performs basic clinical procedures such as Phlebotomy, IV insertion, Wound care, Suturing, Splinting, EKG, administering medications, Incision and Drainage etc.
  • Discusses and reviews cases with Extensivist Physician
  • Formulates and documents Plan of Care in Electronic Medical Record (EMR)
  • Prescribes medication or other forms of treatment for a Patient's diseases and conditions
Renders provision of care either in a clinic based setting or remotely including but not limited to:
  • Starts IVs
  • Administers IM injection
  • Administers Medications
  • Phlebotomy / Lab draws
  • Vitals
  • Administers Vaccinations
  • Performs EKG
  • Blood Glucose Monitor
  • INR Checks
  • Spirometry
  • Breathing/Nebulizer Treatments
  • Ankle Brachial Index
  • Performs Incision & Drainage
  • ACP Discussion
  • Joint Injections (if trained)
  • Performs Suture Repair
  • Splinting
  • Keeps PCP updated on Patient status and actions taken
  • Collaborates with Specialists where clinically required
  • Participates in On-Call program which may include after-hours, weekends, holiday calls, and visitations

Supervisory Responsibility:
  • No direct reports
  • APRN is to report to Practice Administrator (Operational/Administrative)

Physical Demands:
Manage and coordinate inbound and outbound telehealth and in-person outreach
Ability to document in Electronic Database systems
This is an active role. Requires actively examining and treating Patients
Frequent bending and lifting

Position Type/ Expected Hours of Work:
  • This is a full-time position and core hours of work and days are Monday to Friday 8:00 a.m. - 5:00 p.m.
  • Extended hours to accommodate Patients urgent and emergent care needs will be expected


Required Education/ Experience:
  • MS in Nursing required
  • Minimum 1 year of experience with clinical emphasis for APRN, preferably in Geriatrics
  • Current unrestricted NP license in state of application
  • Must have a valid DEA license
  • Must possess current CPR certification (BLS, ACLS, and PALS)

Additional Eligibility and Qualifications:
  • Ability to use Electronic Medical Record (EMR) system to document *Patient encounters
  • Knowledge of ICD-10 Coding and STAR Measures attainment
  • Effective communication and interpersonal skills
  • Valid Driver's License
