Our Uglow Clinic located Dallas, Oregon, offers a small-town atmosphere with close proximity to Salem. This This full-time position requires 32 hours of patient-facing time with a flexible 4-day work week and opportunities for extended hours. Your Role With Us:

Join Salem Health and Clinics as an APP, where you'll play a crucial role in delivering comprehensive primary care to our community in the Willamette Valley. We're seeking dedicated and compassionate professionals to join our team across multiple clinic locations, all utilizing Epic EMR. This role offers flexibility and a supportive work environment, allowing you to focus on providing the best possible care to your patients.

Our patients receive comprehensive medical services through our robust medical group which includes more than 250 providers and 16 specialties, focusing on patient experience and high levels of medical care. Salem is uniquely positioned geographically to serve a broad catchment area including metropolitan and rural communities and boasts active lifestyle opportunities unique to the Pacific Northwest.

Background/Expertise:

You may be a qualified Physician Assistant candidate if you:

  • Have graduated from a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or its predecessors.
  • Have passed the Physician Assistant National Certifying Examination and are certified by the National Commission on Certification of Physician Assistants.
  • Current Physician Assistant license in good standing issued by the Oregon Medical Board.
  • Hold DEA certification.
  • Successful completion of the Physician Assistant National Certifying Examination.
  • Current certification by the National Commission on Certification of Physician Assistants.

You may be a qualified Nurse Practitioner candidate if you:

  • Possess a Master’s Degree or higher in Nursing, or in a field deemed to be equivalent by the Oregon Board of Nursing.
  • Have satisfactorily completed a Nurse Practitioner Program recognized by the Oregon Board of Nursing.
  • Hold a current, unencumbered Registered Nurse license in the State of Oregon.
  • Hold valid, unencumbered Nurse Practitioner national certification issued American Nurses Credentialing Center (ANCC) or National Certification Corporation (NCC), or an equivalent Medicare/Medicaid approved certifying body or be actively seeking certification and obtain the same in the specialty area requested within 24 months of privileges being granted at Salem Hospital.
  • Hold DEA certification.
