We need PA - Family medicine and PA for Pediatrics.

End Date: Ongoing

Only allow bids from providers with active licenses in CA? Yes
Estimated Credentialing Timeframe (Days): 60
The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer
or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).

Qualifications

Minimum Board Certification: Eligible
APC's with 3 years of experience in similar setting
PAs must have CA Controlled Substance Education Course Certificate
NPs must have Furnishing License with Schedules II
Active DEA needed before start

Working Hours

Chart closure within 72 hours
Schedule = 18 PPD, minimum is 18 PPD
Monday-Friday 8-5
Mix of peds/adults
A decision is typically made within 7-14 days
