Bon Secours Patterson Avenue Family Practice - Adding Exceptional Family Nurse Practitioner to the team

Our team of 10 providers - 7 MD and 3 NP - are looking for an exceptional Nurse Practitioner who can handle a high volume practice with minimal oversight. The need is for a seasoned/experienced primary care provider who can start, ramp up quickly, take over a panel and increase our patient access in this premier practice. Primary Care practice experience of at least 2 years is required.

Patterson Avenue is a "Primary Care First" practice - one of only 960 in the U.S. - a value-based care model which prioritizes outcomes and access to care.

• Outpatient Ambulatory Primary care NP practice experience highly preferred
• FNP only - Must be able to treat all ages - peds age 5+ through geriatric
• 15% peds, 50% adult, 35% geriatric
• Schedule is full time - M-F 8a-5p
• Call rotation is shared by practice providers - 1 day out of every 13 days - Rx refills on call are provider discretion, no scheduled drugs or pain meds
• 18-20 patients per day average
• Telemedicine and E-visits will be a part of your practice
• Basic in-office procedures incl. women's health, joint injections, I&D, occasional sutures
• EPIC EMR
• X-ray and Lab in practice
• LCSW, RN Care Manager, PharmD, Community Health Worker, Radiology and Phlebotomist in practice.
• Bon Secours Rheumatology and Physical Therapy in building
• MyChart for telehealth and virtual e-visits
• Seeking a self-sufficient, confident provider, good communicator, asks questions, manages patients' quality outcomes
