COMPENSATION:
- $125,000 to $195,000 Base Salary, dependent upon credentials and experience

POSITION DESCRIPTION:
The Stroke Program Nurse Navigator / Nurse Practitioner reports to the Stroke Program Coordinator and provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members, and support services.
Specific duties of the role include, but are not limited to:
- coordinate multidisciplinary care
- provide stroke education
- provide delivery and oversight of patient care
- post-discharge follow-up
- work in collaboration with key stakeholders to meet program certification requirements
- review data and performance improvement projects
- incorporate the patient experience in all aspects of patient care
The organization uses an EHR.
REQUIREMENTS:
- California NP and RN licenses
- DEA
- NIHSS and ASLS (okay to get within 3 months of hire)
- ENLS (okay to get within 6 months of hire)
- CNRN or NVRN credentials
- SCRN preferred, not required
- new graduate NPs with RN backgrounds in stroke care are welcome to apply
