Job Responsibilities
- Evaluate and care for patients with preventative guidance
- Evaluate patient medical history and conduct physical examinations
- Order and interpret diagnostic tests
- Provide treatment and management plans for patients.
- Collaborate with other healthcare professionals to coordinate patient care
- Educate patients and their families on disease prevention and management
- Offer patients additional ongoing support as applicable such as embedded Behavioural Health, Registered Dietitian, RPM, and Patient Engagement Outreach.
Qualifications
NYS Nurse Practitioner license
