Post-Acute Care | Full-Time | New London, Connecticut

We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our post-acute care team in New London, Connecticut. This full-time opportunity offers strong earning potential, schedule flexibility, comprehensive benefits, and the support of a well-established clinical infrastructure.

Providers in this role typically see 15–20 patients per day while delivering high-quality, patient-centered care across post-acute settings.

Compensation & Benefits

Our compensation model is productivity-based (Fee for Service), giving you control over your income potential.

  • Estimated annual income: $134,000 – $158,000

  • Uncapped productivity bonus potential

Responsibilities

  • Deliver high-quality, safe, and cost-effective care in post-acute settings

  • Provide inpatient rounding in:

    • Skilled Nursing Facilities (SNF)

    • Inpatient Rehabilitation Facilities (IRF)

    • Long-Term Care Hospitals (LTACH)

  • Collaborate with interdisciplinary care teams in accordance with CMS guidelines

  • Coordinate care transitions with hospice, palliative care, and home health agencies

  • Ensure timely documentation and billing in compliance with Medicare guidelines using gEHRimed EMR

Qualifications

  • Board Certified: FNP, AGACNP, ACNP, AGPCNP, or PA-C

  • Licensed (or eligible for licensure) in Connecticut

  • Experience in post-acute or inpatient settings preferred

  • Willing to train experienced candidates transitioning into post-acute care

  • Strong communication skills and collaborative mindset
