Job description
We are seeking a skilled and compassionate Physician Assistant and Nurse Practitioner to join our Cardiovascular Team. This is a full-time, on-site position based in Henderson and Oxford, NC, with potential coverage in surrounding locations. The ideal candidate will provide high-quality care to adults and elderly patients, manage chronic illnesses, and promote preventive health measures.
Job Responsibilities
Diagnose and treat acute and chronic illnesses, including diabetes, hypertension, CAD and vascular diseases.
Develop and implement personalized treatment plans for elderly patients with complex cardiovascular conditions.
Provide preventive care, including vaccinations, screenings, and lifestyle counseling.
Coordinate care with specialists, physical therapists, and social workers as needed.
Educate patients and caregivers on disease management and health maintenance.
Maintain accurate and comprehensive medical records, ensuring compliance with HIPAA and other regulatory standards.
Respond to patient inquiries and emergencies, including in-home care visits if required.
Stay informed about advancements in cardiology and vascular medicine, incorporating evidence-based practices into care delivery.
Participate in quality improvement initiatives and team meetings.
Qualifications
Licensed as a Physician Assistant (PA-C preferred) and Nurse Practitioner in North Carolina.
At least 1 year of experience in Cardiolgy (new graduates with a strong interest in geriatrics are encouraged to apply).
Proficiency in managing chronic conditions and coordinating care for geriatric patients.
Familiarity with EMR systems and ICD-10 coding.
Excellent communication and interpersonal skills.
Empathy and patience when working with patients and their families.
Commitment to continuous learning and professional development.
