Job description
We are seeking an advanced practice clinician to join a growing post-acute care team in the Syracuse, New York area.
In this full-time role, you’ll provide cardiology consults to adult patients in skilled nursing facilities. With no after-hours or weekend call, you’ll enjoy a consistent Monday–Friday schedule that allows you to focus on patient care and maintain work-life balance.
What You’ll Do:

Conduct cardiology consults in post-acute care settings

Collaborate with facility staff and interdisciplinary teams

Monitor and manage chronic cardiac conditions in partnership with supervising physicians

Contribute to quality care initiatives and clinical improvement efforts

Document using a mobile EMR system, with full training and support provided
Who We’re Looking For:

Nurse practitioners or physician assistants with an active New York license

Active DEA license (or ability to obtain)

At least 2 years of experience in cardiology or post-acute care preferred

Strong communication skills and a collaborative, patient-centered mindset

New graduates with relevant training and a passion for cardiology are encouraged to apply—additional training is available
