Case Manager RNs are responsible for the following:

Advocates for the patient and family throughout the entire episode of care.

Monitors patient’s progression intervening as needed to ensure efficient patient progress.

Coordinates all aspects of discharge planning and establishes a safe discharge plan.

The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care. Qualifications for this position include:

Bachelors of Nursing required

Three (3) + years case management/discharge planning experience

CCM certification preferred

Active RN license in NJ/PA
