Job description
We are seeking a Remote Care Manager Nurse Practitioner to oversee care management and ensure the delivery of high-quality care for participants. This role leverages specialized clinical knowledge to review assessments completed by field staff and coordinate comprehensive, individualized care plans that address all domains of care.
Compensation: $75-85 Per Hour
Key Responsibilities:
- Review participant assessments submitted by field staff to ensure accuracy and completeness
- Develop and coordinate holistic care plans tailored to participants’ medical, behavioral, and social needs
- Monitor and evaluate quality of care, identifying gaps and recommending interventions
- Serve as a clinical resource for field staff, providing guidance and support
- Collaborate with interdisciplinary teams to ensure continuity and coordination of care
- Document care management activities according to regulatory and organizational standards
Schedule:
- Monday – Friday, 8:30 AM – 5:00 PM (1-hour lunch)
- Fully remote position
Qualifications:
- Active Nurse Practitioner license in relevant state(s)
- Strong clinical knowledge and experience in care management or case management
- Excellent communication, coordination, and problem-solving skills
- Ability to work independently in a remote setting while collaborating effectively with field teams
