A. Total Number of FTE’s needed? 1 FTE
B. EMR: Epic
C. Coverage Dates – asap pending credentialing
D. Shift Schedule Details & Shift Duration: 5 -8s , 4-10s, or 2-8s and 2-12’s **Guaranteed Work Week = 36 hours
E. Is call required? No call, no weekends
F. Rotation: Monday - Friday
G. Coverage Dates – asap pending credentialing
H. Shift Schedule Details & Shift Duration: 5 -8s , 4-10s, or 2-8s and 2-12’s **Guaranteed Work Week = 36 hours

REQUIREMENTS:
- Minimum length of commitment: Full-time only strongly preferred; Will review PT candidates who are presented as a package deal
- Credentialing Timeframe: 6-8 weeks
- Temps Available? (Requirements for temps, timeframe): locums temps are available but will be the timeframe above. Possibility to credential faster.
- Life Certifications/Qualifications required: ACLS + BLS
- PA License ; Active License required
- Miscellaneous Information: PA Child Abuse requirement for license
