- Start Date: Provider must specify what date in April they are available to start
- Schedule: Monday–Friday, Four 10-hour shifts per week, 7:00 a.m. – 3:00 p.m.
- No call / No weekends
- Any dates unavailable in the next 3–6 months
- Any holidays in the next 3–6 months the provider is willing to work
- Practice Model: Mix of supervision and independent practice
- Majority Case Types: General Surgery, Ortho, GI, Robotics
- EMR: EPIC
- Certifications: FL license, active out of state drivers license, NBCRNA, ACLS, BLS, PALS, BC
- Credentialing: 120 days
- Out-of-state providers only
- Must have a 100% clean file
