Requirements:
- Master's or Doctoral degree in Nurse Anesthesia from a COA-accredited institution.
- Current, active, and unrestricted license to practice as a CRNA in any U.S. state or territory.
- Current certification by the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA)
- BLS (AHA)
- ACLS (AHA)
- NPI
- A minimum of one (1) year direct patient care experience within the last three (3) years. Please note: Providers who have ever had a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
