    • Board Certified or Board Eligible
    • Minimum of 2 years experience
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in-state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
