JCH&L is a 17-bed Critical Access Hospital with a Rural Health Clinic, offering a wide range of services including inpatient and outpatient care, emergency, surgery, diagnostics, therapy, long-term care, specialty outreach clinic, home health and on-site fitness center. At Jefferson Community Health & Life (JCH&L), we’re proud of our long-standing commitment to delivering exceptional care to our community. What began in 1963 as a local vision to enhance healthcare access has grown into a regional leader in rural health services.
Founded with strong community support, JCH&L opened its doors as a 28-bed hospital and 33-bed nursing home. Since then, we’ve continuously evolved to meet the changing needs of our patients. From expanding surgical and emergency services to becoming a designated Critical Access Hospital, we have invested in the people, technology, and facilities needed to provide top-tier care.

A modern up to date 17-bed Critical Access Hospital
Rural health clinic (built in 2022)
Outpatient Specialty Clinic

Attached Long-term care and assisted living services
