Hamilton Medical Consultants, LLC was established by its founding partners, Drs. Anzaldua and Bustamante, as a way to effectively and efficiently provide primary-care, specialty-care, and long-term acute care services at facilities in Central Indiana.
Daniel A. Anzaldua, M.D.
Born and raised in Texas, Dr. Daniel Anzaldua has called the Indianapolis area home since 2003. His undergraduate training was completed at St. Mary’s University in San Antonio, Texas. He then attended medical school at Saba University School of Medicine.
Dr. Anzaldua completed his Internal Medicine residency training at St. Vincent Hospital in Indianapolis. After residency, he did his General and Interventional Nephrology Fellowship at Saint Louis University in St. Louis, Missouri. After completing his training, he provided specialized locum tenens nephrology services to rural and veteran’s hospitals across the country.
Dr. Anzaldua now focuses on providing high-quality care to patients in Indiana.
Peter J. Bustamante, M.D.
Born and raised in Michigan, Dr. Peter Bustamante attended Indiana University Bloomington as an undergraduate. He then obtained a Master’s Degree in basic science research at Loyola University of Chicago followed by medical school at Saint Louis University School of Medicine.
Dr. Bustamante returned to Indiana for Internal Medicine residency training at St. Vincent Hospital & Health Services. He went on to complete his Nephrology Fellowship at Indiana University and Interventional Nephrology training at Saint Louis University and is board certified in Internal Medicine and Nephrology by the ABIM. He also holds Vascular Access Certification from the ASDIN.
Dr. Bustamante has been practicing General and Interventional Nephrology in Indiana since 2008. In partnership with Dr. Daniel Anzaldua, the two started Hamilton Nephrology, LLC to serve the needs of patients with kidney disease and hypertension in central Indiana. In addition, they founded Hamilton Medical Consultants, LLC to focus on post-acute care medicine. Working with key partners in healthcare, they have developed and implemented novel systems and approaches to improve the care of patients following hospital discharge. Their efforts have resulted in a dramatic reduction in hospital re-admissions and improved the care of post-acute patients.
