Alegis Care/Home Physicians provides a uniques opportunity not found in a typical office practice. Our providers work Monday through Friday during the day. There is never a weekend or evening appointment. Our call schedule is also very minimal. We are able to provide our physicians a quality of life not seen in a traditional practice.

Company History
From the corporate headquarters of Home Physicians in Chicago, Illinois, we have been caring for patients since 1995, when we quickly discovered an unmet need for quality home healthcare. We’ve since grown to meet that demand by expanding our management and technological capabilities, and adding comprehensive health services.
Today, we are a multi-specialty, medical health services organization dedicated to creating a comfortable medical home experience for homebound Medicare and Medicaid patients. Through our various care management programs, we deliver specialized, low-cost services ranging from direct patient care for the chronically ill, transitional care from hospital to home and targeted chronic care management programs to comprehensive health assessment services for health plans. We focus on providing primary care/internal medicine, podiatry, wound debridement, and coordinating diagnostic testing, pharmacy and home health services, and durable medical equipment (DME), all in the comfort of a patient’s place of residence. At the heart of Alegis Care services, our highly trained physicians and Providers utilize proven clinical pathways and medication formularies that reduce preventable hospital admissions and healthcare costs, ultimately delivering improved outcomes for our patients and their health plans.
Our current service areas include Illinois, Delaware, Indiana, Maryland, Michigan, New Jersey, Pennslyvania, Utah, Washington, Washington, D.C., and Wisconsin. From our start with a few home physicians in Chicago to now more than 80 Providers across the country, we are continuously expanding our services and geographic reach, further sharpening our focus and promise to deliver the highest quality home healthcare.
