Location: Stockton (2-3 locations all within 5 mins of each other, provider must be okay with going to all clinics)

Start Date: Asap or 2 weeks from interview

Length of Assignment: 3 months w/ possible extension

Schedule: Mon - Fri 8-5pm, 9-6pm, 11-8pm (required once a week), Sat 8-4:30pm (One Saturday a month is required) (cannot do 4-10's at this location, only 5-8's)

Credentialing: 2 weeks, facility reaches out directly for references and will need these completed before start date.

Types of patients: Preventable Health, all ages, pap smear

Patient Volume: 16-20/day (4 hours of admin time / week)

Setting: Primary Care Clinic Setting, Adults, Peds, Women’s Health.
