Where: Spanish Fork, UT

Need: NP/PA

When: ASAP - 4/25 for 30 hours a week.

Shift: Mon and Thurs (12-5:30pm) and Tues and Wed (7am-5:30pm)

Duties: Primary Care, Occ Med, Urgent Care

REQUIREMENTS:

Active Utah license and Utah Dispensing license

Active DEA

Experienced with ages 2+
