As a Home Bound Nurse Practitioner at Doral Health & Wellness, you will play a vital role in delivering quality healthcare services to our patients who are homebound due to their medical conditions. You will work closely with our multidisciplinary team to ensure that patients receive the compassionate care they need in the comfort and familiarity of their homes.

Key Responsibilities:

• Conduct comprehensive health assessments for homebound patients.
• Develop and implement individualized care plans.
• Administer treatments, medications, and therapies as needed.
• Monitor patients' conditions and make necessary adjustments to their care plans.
• Collaborate with a team of healthcare professionals to provide holistic care.
• Ensure that patients and their families are well-informed about their conditions and treatment options.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Current Nurse Practitioner license in New York.
• Experience in home healthcare or a related field is preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.

Compensation:

• The pay rate is $150 - $175 per visit.
