GENERAL SUMMARY OF DUTIES:
Provides comprehensive primary health care including health promotion and disease prevention activities for CHS patients. Responsible for the provision of quality health care for acute and chronic medical problems; follows hospitalized patients as appropriate, to assure continuity of care. Refer patent to appropriate specialists and consultants for diagnosis and treatment procedures when indicated. Generally acts as a health advocate for assigned patients.

ESSENTIAL FUNCTIONS:
1. Provides direct health care to adult patients presenting for care at CHS with or without appointments.
2. Where indicated, obtains complete medical history and physical data on patients. Establishes diagnosis. Reviews patient’s records.
3. Performs clinical services to patients who present with illness or disease.
4. Orders appropriate laboratory and diagnostic procedures.
5. Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
6. Instructs patient regarding medications and treatment instructions. Provides patient education.
7. Adheres to established clinical protocols and procedures.
8. Maintains adequate and legible patient medical records in the established chart.
9. Supervise or participate in patient emergencies as required.
10. Observe all infection control procedures.
11. Follow established health center procedures for patient billing and reimbursement.
12. Participate in quality assurance and other committee activities as required.
13. Provides professional supervision of assigned staff.
14. Participate in staff medical education programs and patient health education activities as required.
15. Maintain professional competence by completing appropriate specialty requirements of CME.
16. Maintain current certifications.
17. Attends required meetings and participates in committees as assigned.
18. Assists with the training of assigned staff.
19. Maintain strictest confidentiality.
20. Participates in professional development activities and maintains professional affiliations.
21. Performs related work as required.
PERFORMANCE REQUIREMENTS: KNOWLEDGE, SKILLS AND ABILITIES
Required:
Knowledge of professional medical practice and care to give and evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of medicine to provide on-going patient care. Skill in taking medical history, assessing medical conditions and interpreting findings. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining department quality assurance. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.
EDUCATION AND EXPERIENCE
Required:
1. Graduate of an accredited medical school.
2. Board certified or Board eligible in appropriate primary care specialty.
3. Licensed to practice medicine in the State of Connecticut.
4. Eligible to obtain and maintain a federal and state narcotic registration.

Preferred:
1. 1-2 years in health care setting.
TYPICAL PHYSICAL DEMANDS:
Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for prolonged periods of time. Occasionally lifts and carries items weighing up to 50 pounds. Requires normal visual acuity and hearing. Requires exposure to communicable diseases and bodily fluids. Requires working under stress in emergency situations or during irregular hours.

TYPICAL WORKING CONDITIONS:
Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations or other conditions common to a health center environment.
