Job Summary

Under the supervision of the Medical Director, provides primary medical care to participants of the InnovAge Greater New Mexico (IGNMP) – Program of All Inclusive Care for the Elderly (PACE), including assessments and development of plans of care in collaboration with the Inter-Disciplinary Team (IDT). The Physician is responsible for the provision of direct patient medical care as well as the evaluation of effectiveness of care given and functions in a collegial relationship with the other health professionals. Makes independent decisions regarding medical needs and health regimens, and carries out medical procedures as indicated.






Essential Functions and Work Responsibilities



Functional Category: Treatment

Estimated Percent of time Spent - (Approximately 50% of Time)

· Provides preventive health care and health promotion for IGNMP participants.

· Treats and manages acute chronic illnesses in the day health center, nursing home, group home, or in participants’ homes.

· Regulates and adjusts medications as needed and in a cost-effective manner.

· Facilitates medical specialist consultation as needed.

· Educates clients and families regarding health maintenance and their chronic medical conditions.

· Collaborates with inpatient attending physicians (hospitalists) when participant is hospitalized or admitted to a nursing home and communicates participant status to IGNMP team for those participants for whom the physician does not assume primary responsibility.

· Works as supervising physician with the nurse practitioner or clinical nurse specialist.

· Oversees specialty and inpatient medical care.



Functional Category: Assessment

Estimated Percent of time Spent - (Approximately 30% of Time)

· Obtains a complete health history and records the findings in a systematic organized manner.

· Orders and evaluates appropriate laboratory and diagnostic tests in a cost-effective manner and records systematically.

· Provides initial and ongoing participant medical assessments, care plan development and implementation of care plan using standardized procedures.

· Evaluates the effectiveness of the participant’s plan of care and revises as appropriate.

· Triages status changes in participants and facilitates appropriate management of problem identified.

· Identifies and describes behavior patterns of the chronically ill.



Functional Category: Additional Duties

Estimated Percent of time Spent - (Approximately 20% of Time)

· Serves on various committees of the organization as requested.

· Attends pertinent health care conferences and courses to maintain knowledge of current trends in geriatric health care.

· Serves in physician on-call rotation as required.

· Works as supervising physician with the nurse practitioner or clinical nurse specialist.

REQUIRED



Education

· Doctor of Medicine or Osteopathy degree required.



Licensure, Certification, Registration or Designation

· Current active New Mesico Medical License

· DEA number

· Adequate malpractice coverage

· Board Certification in Internal Medicine or Family Practice with advanced certification in geriatrics preferred



Work Experience and Qualifications

· A minimum of one year primary care experience with the frail or elderly is required

· Must have the ability to obtain and maintain staff privileges at TCC contracted hospitals.



PREFERRED



Licensure, Certification, Registration or Designation

· Advanced certification in geriatrics preferred




Other Knowledge Skills and Abilities Required



Computer Skills

· Must be computer proficient and possess experience with Microsoft Word, Excel, and Outlook.

· Must be able to quickly learn specific software and new applications.



Mathematical/Financial Skills

· Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

· Able to analyze data and statistics and draw reasonable conclusions and compile accurate reports.

· Experience with P/L and developing and managing budgets.



Language Skills

· Ability to read, analyze and interpret regulations and other documents.

· Strong interpersonal skills and ability to effectively and tactfully present information to, and communicate with, co-workers, employees, and others.

· Possess exceptional English written and verbal communication skills, including accurate grammar and business correspondence knowledge.

· Ability to read and write memos, reports, and correspondence that conform to prescribed style and format.
