A facility in the Salt Lake Valley in need of an NP preferably but will consider PA's to help in oncology.
- APRN partner passed away in October, needs to hire someone as a replacement.
- Prefer NP, open to PA’s
- Schedule Needed: Work per patient
- Flexible or fixed days
24-hours/week December forward
38-hours/week January forward - Review the medical record 1-hour (can be done remotely)
- Face to face meeting with patient, 30 min - 1 hour
- Create Chronic Care Management (CCM) Plan, 30 min (can be done remotely)
- Flexible or fixed days
- Outpatient
- Complex Chronic Care Management -Oncology
- Preferred Certification: OCN - Oncology Certified Nurse; CBCN - Certified Breast Care Nurse
- EMR: Not needed, outsourced to other clinics
- Privileging Time Frame: 2-3 weeks
