- Coverage Needed:
- August 6, 2023- ongoing
- Schedule: Monday " Friday w/ occasional Weekend Call Coverage
- Hours: 8 AM " 5 PM
- Call: Q4 Days
- On Call: 5PM- 8AM
- Clinical Information:
- Inpatient: Rounding, OR Cases, On-Call Cases
- Outpatient: Office Visits, In-Office Procedures
- Supporting staff: LVHN to provide in-office staffing support.
- Volume: Office: 28-30 patients per day.
- OR- Case Length dependent
- Required:
- DEA must remain in state to maintain privileges.? Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
- Exemptions only allowed for religious or medical reasons, once approved by the facility.
- Board certified in Urology
- Procedures required: Urologic Surgery- ESWL, Lithotripsy, Stent Placement, Stone Removal, BPH Procedures, TURBT
