Their focus is on providing comprehensive care to patients at home, improving outcomes and reducing healthcare costs. They operate nationwide, serving a variety of Medicare Advantage, Medicaid, and commercial clients. Among other offerings, their in-home care program includes primary care, health risk assessments, transitional care, post-acute care, sleep management, and home infusion therapies. They are currently hiring due to growth in their Philadelphia market.
POSITION DESCRIPTION:
Providers are supported by fully-trained scheduling and administrative team members who assist with triaging phone calls, making referrals, etc. Patients are seen in facility settings as well as via house calls. Providers can work via telemedicine from home one (1) day per week, and the practice is flexible on the day.
The overall duties of the physician role include:
- provide decision support to nurse practitioners and collaborate with the multidisciplinary team
- perform preventive visits to optimize chronic conditions
- assess home environments
- develop proactive care plans
- educate patients/family members on medical conditions, preventative care, and medications
- perform urgent care visits to avoid unnecessary ED visits and admissions
- co-manage patients with PCPs and serve as an extension of clinical care in the home
- assume responsibility as the home-based PCP in cases where there is no PCP
- perform telehealth visits when needed
- function as the day-to-day clinical leader
The new physician will work within the areas of comprehensive health risk assessments (HRA), the complex care program (CCP), and overall chronic care management (CCM).
HRA:
- conduct in-home comprehensive health assessments
- work to ensure patients’ medical well-being
- provide accurate data for hierarchical condition category coding, etc.
CCP:
- conduct in-home longitudinal care for medically complex patients
- work to improve patients’ clinical outcomes
- collaborate with patient PCPs, serving as an adjunct to their care delivery
CCM:
- assume primary care of chronically ill patients with high admission rates
- work to improve patients’ clinical outcomes
- work alongside nurse practitioners to deliver care in patient homes
Providers utilize a mobile EHR and communicate with their support team remotely.
REQUIREMENTS:
- Pennsylvania medical license
- DEA
- Board Certification, Family Practice or Internal Medicine
- BLS certification
- 2+ years practice experience
- full COVID vaccination required
