* Established Kidney Transplant Program in Transplant clinic
* 396 bed tertiary care center in Ft. Wayne, IN which is part of Health Network
* 8 hospital system across northeastern Indiana
* Specialty services include: Transplant trained nursing staff Post-transplant monitoring and outpatient clinic, Living donor transplant team, Transplant outreach coordinator, Interdisciplinary team of dietitians, financial counselors, referral coordinators, social workers, and physicians works together to develop a customized patient care plan.
