- Active TX license and DEA
- Clean background and malpractice for temps
- BC
- ATLS, BLS
- Send self-query NPDB and EPA with presentation
- Must submit vaccination card with presentation
- Must provide Case Logs: This will be required even for disaster temps, must list physician name, facility
- name, procedure name (not just codes), quantities of each and date range of log or dates
- performed – must be in the setting in which they are applying (ICU/Critical Care or HM
- Inpatient or EM) and show patient activity in the past 12 months – request these ASAP as
- these take awhile to obtain. We can get a release signed form the physician to assist in getting these.
