About:

Established in 1989 by two graduate doctors. Later, in 1991 growth began within the practice by adding two new doctors.

In 1994 the practice grew to accommodate separate well and urgent care areas and in 1995 came the introduction of our first mid-level providers (physician’s assistant and Nurse Practitioners). By 1999 the growth of the practice had expanded by adding more mid-level providers.

In September of 2005, we moved into a new facility which allows us growth for the coming years.

Salary – negotiable. . Employee reviews are conducted annually with a potential cash bonus of up to 5% of the physician’s total salary.
Work Schedule/Patient Load –The physician shall work 4 days a week, one inpatient day, three outpatient days (2 days- 8am to 5pm, 1 day- 8am to 7pm), one night of call per week and one weekend of call per 5 weeks. Depending upon unforeseen provider staffing changes, the employer may need to adjust this schedule from time to time. The physician shall see approximately 20 patients a day.
Bank Time – The Physician accrues 4 weeks of bank time per year. Bank time includes vacation and personal time. One week of CME time per year will be granted at the beginning of each year and does not roll over to the following year. We grant 8 paid holidays after 30 calendar days of service. We grant paid vacations and CME leave after 90 calendar days of service.
Malpractice Insurance – The employer pays for the employee’s malpractice insurance premiums. The tail coverage is prorated dependent on the years of service and is included in the physician contract.
Continuing Education Expenses – Reimbursement of CME’s expenses as outlined in IRS code of up to $2,000.00 per year.
Professional Dues and Licensure Payments – The employer pays for all professional dues and licensure fees.
Health Insurance - 3 Health Insurance Plan Options
$1,500 deductible - we pays 65% of this insurance premium
$4,000 deductible - we pays 75% of this insurance premium
$6,500 deductible HSA - we pays 85% of this insurance premium
Flexible Spending Account and Dependent Care Reimbursement Account - A plan that enables you to save pre-tax dollars to pay for medical expenses and dependent care expenses.
Short Term Disability - The employee receives 60% of wages up to $1,000 maximum per week for 26 weeks. The employer pays 80% of short-term disability and life insurance premiums.
Group Term Life Insurance - The employee receives life insurance of 2 times the employee’s annual salary with a $250,000 maximum. The employer pays 80% of life insurance premiums.
Dental Insurance - The employer pays 80% of dental insurance premiums.
Vision Insurance-The employer pays 50% of the insurance premiums.
401(k) – Employee must meet eligibility requirement of 90 days of service and may sign up on the 1st of the following month depending upon date of hire. Profit sharing contribution is discretionary and is to be determined by the employer. Currently, We contribute 4% of the employee salary when they contribute 5% or more.
Voluntary Insurance through American Fidelity - Cancer Plan, Accident Plan, Hospital Indemnity Plan, Long Term Disability, Term Life Insurance, Whole Life Insurance. Employee pays 100% of the premium.
