* Employed Position
* Base salary of $680,000 with performance bonus
* $75K sign-on bonus
* Potential performance bonus up to $80K
* $7K annually for dues, subscriptions and CME allowance
* 26 days away from the practice for holidays, personal days, vacation, sick and CME
