Credentialing Timeframe: ~60 days
• Temps Available: no – could possible expedite a completely clean file
• Total Number of FTE’s needed? 1.0
• EMR: EPIC – must have experience
• Coverage Dates: Need is for September forward, but realistic start with credentialing is 10/22/20 and need is for 3-6 months; possible extensions
• Shift Schedule Details & Shift Duration: Monday – Friday, 8a-5p.
• Call Schedule/Responsibilities: No call
• Type of Patients/Cases: Adult outpatient clinic cases
• Patients per Day/Volume: 18 patients per day
• Practice Setting: Outpatient clinic
• Support Staff: Nurses and office staff – will work solo as physician
• Number of Beds: n/a
• Trauma Level: n/a
• Code Response (Types and who handles): n/a
• Miscellaneous Information: *Clinic is open to a (consecutive and consistent) 3-4 day work week, if needed.
• What is the minimum length of commitment requirement? 3-6 months; full time candidates will be given priority over any part time availability
• Will a client review a Candidate if they do not have full time availability? Will review, but will pass on all part time applicants in favor of full time candidates. Would need a consistent half-time to present.
• Certifications/Qualifications Required: BLS
• IMLC State: Yes
• Active State License Required Or IMLC in hand. Active license is preferred
• DEA Required; Requirements: BC/BE: Board Certified,Board Eligible
