Job Description
Responsible for the quality of patient centered care delivered; delivers medical and clinical reports to organizations management team on clinical issues that affect patient care delivery; serves as a clinical resource to physicians, mid-levels and nursing staff; functions in accordance with policies and procedures established by the board of Directors and performs duties within the medical setting as a member of the care team; responsible for attendance and punctuality to ensure the clinic is operated in an efficient and cost-effective manner; supports and promotes all activities of a patient centered medical home |
Essential Functions: · Provides care for patients in a patient centered mode, commensurate with training, experience, and clinic needs, to include well-check visits, acute illness visits, family planning, etc. · Maintains timely and accurate electronic health record documentation on all patients including office visits, telephone calls, and hospital admissions/visits, if applicable · Prepares timely and accurate billing charges; completes and closes tasks within company policy · Makes appropriate referrals to other health facilities for further testing, diagnosis, treatment, and follow-up care · Manages patient utilization of services · Participates in daily huddles · Participates in accreditation agency on-going readiness · Participates in Peer Review and all relevant Quality Assessment/Quality Improvement activities, as assigned · Strives to achieve goals of the health care plan and all assigned clinical goals · Supervision and teaching of residents, medical students, and other allied health professionals, as assigned · May be required to work in multiple locations · Responsible for supervising the provision of clinical care by mid-levels, laboratory and nursing staff · Participate in 24-hour telephone/afterhours coverage, as assigned |
General Duties: · Support the Mission, Vision and Values of Tampa Family Health Centers (TFHC) · Participate in QI related activities, with a focus on providing quality care and closing care gaps · Is accountable to ensure patient rights and ensures an environment that promotes the privacy, dignity and well-being of all patients in a safe, secure environment · Supports and participates in common teamwork; fosters a culture of teamwork and cooperation · Promote positive public relations with patients, visitors and community · Effectively and efficiently completes all duties as assigned · Adhere to all HIPAA, State, Federal, accreditation agency, HRSA and TFHC rules and regulations |
Knowledge and Experience: · Able to develop and maintain positive, professional working relationships with patients, care team members, other medical providers and vendors · Able to relate with people with a wide diversity of ethnic origins, economic status, religious affiliations and gender orientation · Strong communication skills, both written and oral · Able to navigate and enter data into an electronic health record |
Education, Certification Training and License: · Attendance and graduation from an accredited medical school · Attendance and graduation form an accredited residency program · Current BLS certification · Current and valid State of Florida Medical License · Current and valid DEA Registration Certificate · Board Certified or Board Eligible in a primary specialty. If Board Eligible, certificate to be obtained within (3) years of employment · Maintain Continuing Education Credits, as required |
