A health services organization in New Jersey is currently seeking a licensed Physician, with a Primary Care background, to join their staff as their new Director of Population Health. In this role, the Director of Population Health will be responsible for overseeing and providing primary and preventive care at the facility.

- Deliver primary care within the practice, through both traditional face-to-face visits as well as non-visit based interactions (e.g. phone and digital messaging)
- Assist patients to understand diagnostic tests, costs, risks, treatment options, preparing for medical appointments, and their health benefits
- Initiate referrals to specialty services and ensures bi-directional communication with specialist to promote continuity of care
- Work collaboratively with other staff to promote team-based care
- Design and implement new programs to achieve improved health outcomes and lower total cost of care, including: statin adherence, wellness screenings, care coordination, reducing ED admissions
- Conduct oversight of quality improvement activities
- Optimize the EMR for clinical workflows and reporting
- Ensure the practice meets customer's goals and contractual measures
- Medical Degree from an Accredited University
- NJS Medical License
- Board Eligible or Board Certified in Internal Medicine or Family Medicine
- Primary Care background
