Job Description

A Palliative Care or Geriatric Medical Director is needed in Newark, California. Join a dedicated team whose primary focus is to provide expert, compassionate, and personalized care throughout the Bay Area.

Details:

• Permanent, full-time position

• Leader for providing comprehensive care coordination for high-risk patients

• Team may include physicians, advanced practitioners, nurses, social workers, pharmacists, and care coordinators

• Participate in determining the best support for patients considering current options available in communities

• Monitor and report outcomes for patients cared in advanced care program

• Provide direct patient care, at least half of the time

• Provide oversight to the clinical team and resources

• EMR: Epic

Qualifications

• Board Certified in Palliative Care or Geriatric Medicine
