Provider Requirements:
- 5 years in medical practice for the Chronic Care population
- 1 year Primary Care experience with a frail or elderly population
- Experience in a lead Medical Administrative role and/or previous experience as a Medical Director
- Experience working with an interdisciplinary team
- BC in Internal Medicine or Family Practice
- Geriatric certification preferred
- Knowledge of chronic care/geriatric issues
- DEA registration
Facility/Job Details:
- Supervises all medical/paramedical services
- 75% Medical Director, 25% Direct Patient Cares
- Day to Day Primary Care
- Oversee 2 Nurse Practitioners
- Coordinates 24-hour care delivery for participants through the sharing of rotating night and weekend on-call duty with other providers
- Oversight of the quality assessment and performance improvement program (QAPI)- care plans
- Development of the yearly QAPI Plan, quarterly and year-end evaluation and reporting, collaborating with quality committee and the board as directed
- Oversees and maintains medical policies, standards and protocols
- Accurate and timely documentation of all services provided through use of EMR
- Positive interpersonal relationships with staff and clients
- Supervises and directs staff reports
- Act as a Liaison between facility and community health agencies’, Physicians, and other health service providers
