Provider Requirements:

  • 5 years in medical practice for the Chronic Care population
  • 1 year Primary Care experience with a frail or elderly population
  • Experience in a lead Medical Administrative role and/or previous experience as a Medical Director
  • Experience working with an interdisciplinary team
  • BC in Internal Medicine or Family Practice
  • Geriatric certification preferred
  • Knowledge of chronic care/geriatric issues
  • DEA registration

Facility/Job Details:

  • Supervises all medical/paramedical services
  • 75% Medical Director, 25% Direct Patient Cares
  • Day to Day Primary Care
  • Oversee 2 Nurse Practitioners
  • Coordinates 24-hour care delivery for participants through the sharing of rotating night and weekend on-call duty with other providers
  • Oversight of the quality assessment and performance improvement program (QAPI)- care plans
  • Development of the yearly QAPI Plan, quarterly and year-end evaluation and reporting, collaborating with quality committee and the board as directed
  • Oversees and maintains medical policies, standards and protocols
  • Accurate and timely documentation of all services provided through use of EMR
  • Positive interpersonal relationships with staff and clients
  • Supervises and directs staff reports
  • Act as a Liaison between facility and community health agencies’, Physicians, and other health service providers
