The Medical Director will be responsible for directing physician clinical services to ensure compliance with local and national policies and standards. The Medical Director will manage a panel of ~200 patients who exhibit complex problems affecting their independence and will assume primary medical responsibility for his/her assigned patients.

Qualifications

Education/Licensing Requirements:

• MD/DO degree

• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine

Experience Requirements:

• Demonstrated clinical competencies in such areas as age-specific care, cultural assessment and health education

Essential Skills/Experience

• Fluently speak, read, and write English.

• Excellent verbal and written communication skills.

• Proven ability to prioritize and multi-task.

• Computer skills: Microsoft Excel, Word, Outlook. Experience in EMR strongly preferred

• Professional demeanor, self-motivated, and dependable.

• Ability to communicate effectively with all personnel levels within the organization.

Essential Job Responsibilities

• Direct physician clinical services to ensure compliance with local and national company and recognized standards and policies as well as accreditation standards.

• Oversight of weekly Interdisciplinary Team Conferences to ensure consistent and clear communications among the entire team.

• Participation with Human Resources for the Practice including selection of qualified staff in collaboration with the Practice Manager, orienting and mentoring the staff, and promoting staff development and professional growth to ensure competency.

• Managing special projects and initiatives as determined by the CEO/CMO.

• Collaborating with leadership on program development and implementation and continuous quality improvement pertaining to practice performance (quality of care, patient satisfaction, utilization, documentation and quality measures).

• Manage a panel of high risk socially complex patients by: o Assuming primary medical responsibility for his/her assigned patients

o Identifying the patients' medical problems

o Defining the medical management of patients' problems

o Determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics

o Determining the need for, and facilitating, admission to the hospital

o Visiting the patients at home, when appropriate and as defined by the population health and clinical guidelines

o Following corportate and Health Plan clinical guidelines of care for patients

o Timely and accurate documentation of all clinical encounters

o Providing medical administrative support such as signing forms requiring physician signature for the patients

o Participation in Multidisciplinary Care Planning Meetings

o Other duties/projects as assigned
