They own and operate outpatient centers, while also partnering with local physician practices to enhance and expand services. They have approximately 500 providers, in a variety of primary care and speciality areas, in their network and are excited to continue expanding access in Arizona.
POSITION DESCRIPTION:
The new physician will work within a well-supported infrastructure that provides 24/7 care access to patients. Each patient is assigned to a care coordinator, who functions as their liaison. When a healthcare concern or question arises, the patient contacts their coordinator (or the after-hours emergency line) to be triaged, book appointments, get specialist referrals, etc.
For physicians on a day-to-day basis, this is a standard outpatient practice setting and the duties of the role include, but are not limited to:
- adult primary care
- geriatric primary care
- chronic disease management
- treatment of minor illnesses and injuries
- specialist referrals
- telehealth visits
The patient base consists of adults/geriatrics and the daily volume varies based upon physician preference and patient needs. Most providers see between 20 and 25 patients per day with the support of a dedicated clinical assistant.
The practice additionally provides in-home health monitoring equipment, which they send to patients free of charge, to allow the office to receive regular diagnostic information on patients’ health conditions, providing an even higher, more responsive, level of care.
The practice uses a customized EHR, developed through eClinicalWorks.

- Arizona medical license
- DEA
