This practice sees patients in private residences and assisted/independent living facilities (ALFs/ILFs) throughout Indianapolis and the surrounding area. The practice works with providers to define territories that are mutually beneficial and ensure comfortable commute times.

Geriatric primary care practice serving patients in Indianapolis and surrounding communities. Care is provided in patient homes and facility settings (ALFs/ILFs). They are currently hiring due to growth.
POSITION DESCRIPTION:
The physician will serve as a primary care provider for a small, dedicated panel of 200 to 225 geriatric patients.
The clinical focus is on:
- preventative medicine
- chronic disease management
- routine geriatric primary care
The office staff performs all scheduling duties, answers and triages phone calls, and supports the physicians however possible in their remote capacity. Physicians communicate with their team via email, phone, and through the EHR.
Physicians see 10 to 12 patients per day, efficiently scheduled by the office team.
Patients are seen as often as deemed necessary by the physician, which is typically 1 visit per month.
Physicians utilize a mobile EHR on a laptop.
There are no evening or weekend responsibilities. On-call is flexible/negotiable - it is phone triage only with low volume.
REQUIREMENTS:
- Indiana medical license
- DEA
- Board Certified or Eligible, primary care field
