VISTA represents the second largest hospital in the state of Vermont seeking a Geriatrician to join their growing, post-acute care program. Candidates must be BC/BE in Family Medicine or Internal Medicine.
Practice Benefits:
* Incoming physician will be based at two local nursing homes
* Full-time position with support from two Nurse Practitioners
* Certified Level III Medical Homes
* FQHC
* Centers provide approximately 80% of the primary care in the service area
* Highly developed and supportive panel management and case management system in place
* Robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and licensed Clinical Social Workers
* Minimal call at one night per month
* EMR and practice management system available
* Benefits include health, dental and vision, short and long-term disability, 403(b) with hospital contribution, relocation, vacation pay, CME allowance and professional liability insurance
