HOURS OF OPERATION:

- Monday through Friday

- 8:00am to 5:00pm



FULL-TIME COMPENSATION:
- $185,000 to $200,000 Base Salary, dependent upon experience

- Quarterly Bonuses (up to $30,000 annually)

LOCATION:
- Greater Cincinnati Area

The hiring managers will work with the physician to determine appropriate practice site(s). Territories are built around providers’ home locations to ensure minimal commutes.


COMPANY PROFILE:
Geriatric primary care practice serving assisted and independent living facilities (ALFs/ILFs), as well as private homes, throughout Ohio and Northern Kentucky. The practice was founded and is led by a physician with 15+ years of experience in the home-based primary care field. They currently employ a team of more than 20 talented physicians and nurse practitioners, who are supported by dedicated clinical and administrative team members. They are currently hiring due to growth.



POSITION DESCRIPTION:
Serve as the primary care provider for a small, dedicated panel of geriatric patients.

The practice believes strongly in proactive medicine, with a clinical focus on preventative care, chronic disease management, medication management, and other geriatric care needs.

Physicians typically see patients on a monthly basis, but this can be adjusted per physician request and based upon patient needs.

Physicians typically travel to 1 or 2 buildings each day, seeing an average of 10 to 11 patients per day. House call visits are scheduled as travel and location allow.

Territories are coordinated around the provider’s home location to ensure minimal drive time.

The patient locations are:

- 70% assisted and independent living facilities

- 30% house calls

Physicians utilize a highly-supported mobile EHR (GEHRIMED) and communicate with their office-based support team via phone, text, email, and EHR.

Providers evenly share on-call responsibilities. The current rotation is 1 week every 4-5 months and this will decrease as additional providers are hired. On-call is phone triage only and volume is typically light, as patients are seen frequently and most have access to facility staff for minor concerns.



REQUIREMENTS:
- Ohio medical license
- DEA
- Board Certification or Eligibility, relevant field
