Position Details:
- Clinician to join our newly redesigned Palliative Care Program
- Plenty of growth and leadership opportunities
- Monday - Friday : 8am - 5pm
- Average Patients Per Day ~10-12 PPD | 60 mins for new patients and 30 min follow up appointments
- Patient Mix : >65 with chronic medical conditions eg. CHF, COPD, Diabetes
- No weekends or nights required but must be open to being on call
- Shared call with all high risk providers, one weekend every fifteen weeks but do not need to be home bound
- Majority of the time will be in an office setting but will do some home, SNF, Hospital visits
- Dedicated support staff : Care Partner or Social Worker assigned to each patient depending on needs
- Support in Clinic : NP, MA, LVN, and Pharmacist to clinic panel size of 300 patients
Requirements:
- Unrestricted California State Medical License
- Current California DEA certificate
- BC/BE in Internal Medicine or Family Medicine
Preferences :
- Critical Care, Palliative, or Geriatrics Boarded MD/DO with previous experience highly preferred
- Previous experience in a managed care setting and working with chronically ill patients
- Medical Spanish preferred but not required
