Hospital has an Advanced Practitioner opportunity in its Critical Care/ICU Unit.
At least 2 years of experience

PURPOSE OF POSITION: To work collaboratively with families and a multidisciplinary health team in an inpatient hospital-based critical care specialty unit with 24/7 coverage including night, overnight, weekend, and or holidays to provide acute/chronic health care to patients, including clinical management for a critical care population. Procedurally based and high acuity patients. Provide leadership through education, consultation and research. This position requires a high degree of confidence in performing duties independently without close physician supervision.

RESPONSIBILITIES:
- Provide first call coverage for the hospitals critical care physician team
- Evaluate patients being admitted to the ICU from the emergency department, hospital wards, and operating rooms
- Formulate a plan of care, request appropriate diagnostic tests, enter appropriate medication, nursing and life-support device orders
- Maintain a high degree of collaboration with the ICU nurses in managing the patients critical care needs
- Communicate with the critical care provider team when deemed necessary

JOB REQUIREMENTS:
Technical Skills: Technical skills for the position include:
- Placement of central venous access
- Placement of arterial lines
- Endotracheal intubation
- Basic ventilator management (the ICU has 24-7 staffing by expert Respiratory Care providers)
- To be lead on code blue status

Assessment and Diagnosis:
- Collects & documents comprehensive data in a systematic & ongoing process pertinent to the patient's health or situation, involving the patient/family/caregiver when appropriate.
- Initiates & interprets age-appropriate & condition-specific laboratory tests & diagnostic procedures.
- Analyzes & prioritizes complex data to determine the differential diagnoses & revises diagnoses as appropriate to initiate the plan of care.
- Assessment & diagnoses derived are developmentally appropriate & specific to areas of growth & development, age, cultural sensitivity, & family dynamics.

Outcomes Identification and Planning:
- Develops outcomes as measurable goals that provide direction for continuity of care & continuity of care & consistency among providers.
- Develops, prioritizes, evaluates & modifies an individualized comprehensive plan of care. Addresses each diagnosis or issue, as appropriate.
- Derives outcomes & plans of care that involve the patient/family/caregiver when possible & are realistic to the patient/family/caregiver capabilities & resources.
- Considers risks/benefits, costs, clinical effectiveness, current scientific evidence/clinical guidelines & clinical expertise to formulate interventions, treatments & outcomes.
- Incorporates new knowledge & strategies to initiate change in practice if desired outcomes not achieved.
- Maintains documentation & updates accordingly outcomes, goals & the plan of care.

Care Management, Education, and Treatment:
- Maintains competency in skills/procedures

- Employs diverse & complex strategies, interventions, & teaching to promote health & a safe environment

- Provides consultation to influence the identified plan of care, to enhance the abilities of others to provide health care, and to effect change in the healthcare system
- Makes appropriate referrals and discusses recommendations with patient/family/caregiver
- Orders and/or performs appropriate treatments, therapies, & procedures that are based on current knowledge, research, & practice
- Provides patient/family/caregiver with diagnostic & laboratory results as well as actual/potential benefits, limitations, & adverse effects of proposed therapies.
- Consistently integrates age specific and culturally diverse concepts into patient care, taking into consideration both the patient's chronological age and level of functioning.

Prescriptive Authority:
- Prescribes appropriate pharmacologic/non-pharmacological agents & interventions that are both universal & unique to the patient, & monitors/evaluates effects.

Evaluation:
- Uses a systematic, ongoing evaluation of the assessment data & outcomes to revise or resolve the diagnoses, outcomes, the plan of care, & the implementation as needed
- Evaluates the accuracy of the diagnosis, effectiveness of the interventions & the plan of care strategies in relation to attainment of expected outcomes
- Includes the patient/family/caregiver & other healthcare providers
- Bases the evaluation process on advanced knowledge, practice, research, & documents appropriately
- Utilizes the results of the evaluation to determine the impact on the patient/family/caregiver, organization, & system to make/recommend process or structural changes including policy, procedure, or protocols as appropriate.

Professional Practice:
- Participates in quality improvement activities or research to improve the care delivery process & patient care outcomes
- Uses & communicates results to initiate change in practice

Required:
- Master's Degree in Nursing AND Current license to practice nursing in the State of New Hampshire
- Certification as an advanced practice nurse appropriate to meet requirements of Nurse Practitioner position (e.g., PNP/A, ) required AND Certificate to prescribe as applicable
- PA Degree
- 3-5 years of critical care experience
