Join this established team of 2 physicians and 4 advanced practice providers; two daytime teams of one physician and two APP’s Full-spectrum of specialtiesMonday through Friday scheduleOversee the care at four skilled nursing facilitiesACLS required

Responsibilities and Requirements of the Post Acute Physician

Responsible for admissions and for executing the post-discharge plan of careResponsible for ensuring safe discharge to home from the skilled nursing facilitiesProvide a true continuum of care to our patients alongside our facility partnersOversees nurse practitioners providing transitional care services in both the home and skilled nursing facilitiesCompetitive compensation, eligible for quality and productivity bonusesBenefits to include medical, dental, vision, 401K, CME allowance, malpractice insurance and moreOur post-acute program will provide extensive administrative and logistical support to the providers as well as portable technology, mileage reimbursement, and flexible scheduling and a 5 day work weekThis is not shift work.
