Job Summary

Serve as primary care Physician to the PACE-Gulf Coast Village Center in Cape Coral & PACE-Charlotte Center in Port Charlotte areas. Overseeing care & treatment rendered by PACE teams as well as providing direct patient care. Hope offers diverse opportunities across our service areas throughout SW Florida. You can make a long lasting and positive impact while providing reliable expertise, compassionate care and providing a high level of customer service.

• Key Responsibilities
  • Provide direct patient care in PACE programs in Cape Coral & Port Charlotte Centers
  • Oversee care & treatment rendered by PACE teams
  • Collaborate as part of an Interdisciplinary Team
  • Carry out supervisory responsibilities
  • Daily travel in service area required

• Required qualifications
  • M.D. or D.O. from Accredited Medical School – Currently licensed & in good standing in FL
  • Board Certified in Family Medicine or Internal Medicine required
  • 1 Year Elder Care experience required
  • Geriatric experience preferred
  • Must possess a valid driver’s license in the state of Florida, current automobile insurance, and reliable transportation.
  • Clear level 2 background check
