Hospice and Palliative Medicine Physician Needed - Fort Wayne, Indiana!!
Hospital, on behalf of a community-based, non-profit agency located in Fort Wayne, IN seeks a Hospice and Palliative Medicine Physician who is either fellowship trained or has at least 2 years of experience in the field. Board certification in hospice and palliative medicine is preferred but not required. The successful candidate will join 1 board certified hospice and palliative medicine physician and 4 nurse practitioners in an established program that covers 8 counties. Our agency provides care to hospice patients in homes, assisted living and nursing facilities, hospitals and our inpatient hospice unit. Our palliative medicine consultation service began in 2000 and has grown to provide consultations in the outpatient setting in a local cancer resource center and advanced cardiac therapy office in addition to several long term care facilities and three local hospitals (one large tertiary facility and two smaller community based).
