Advanced Practice Provider (NP preferred) with Gastroenterology experience needed for growing ambulatory sites in Long Island:

  • 100% outpatient primary care practice
  • Built-in referral system and insurance network for primary care referrals
  • This NP will focus on the GI/Gastroenterology practice in an ambulatory setting
  • Specialty coverage and hospital affiliation for specialty services not provided on-site
  • Full scope of primary care services: diagnosis, treat, coordinate patient care, preventive care

  • Prescribe medications and formulate ongoing treatment and disease management plans

  • Focus on preventive health recommendations and habits

  • Relationship-centered care, collaborative model: holistic, psychosocial, and physical needs

  • Providers see 18-25 patients per day
  • Each site when fully staffed: 2 MDs, 3 RNs, 2 Mas, 1 APP
  • Providers generally hold standard business hours + one-two later evenings per week (6pm-8pm), and one rotated Saturday per month

  • 1+ years in a Primary Care setting
  • 1+ years in a Gastroenterology setting
  • Registered Nurse - NYS Active License (APRN)
  • Excellent communicator - PCMH/Collaborative Care Model
  • Knowledge in team-based care
