Our client, a nonprofit healthcare organization in the New York Metropolitan Area, is currently seeking a Primary Care Physician. The Primary Care Physician will provide care and treatment to the center’s patients.
Responsibilities:
- Provide clinical supervision for all medical / clinical staff, nurse practitioners, and physician assistants
- Provide back-up medical coverage for medical / clinical staff
- Lead the Patient-Centered Medical Home (PCMH) team
- Represent the CMO at state and national levels when requested
- Participate as a member of committees, including peer review
- Perform history and physical assessments
- Provide services in accordance with generally acceptable standards of medical practice
- Develop a plan of treatment/care for the patient that also includes education and follow-up visits
- Make appropriate professional referrals, as indicated by the needs of the patient
- Communicate with other health care providers related to diagnosis, treatment and plan for the patient
- Maintain current and complete documentation in the medical record that reflects the care and services provided to the patient
- Adhere to the center’s established policies and procedures
- Participate as a member of the medical staff and adheres to the medical staff bylaws
- Supervise and consult with staff nurse practitioners/physician assistants
- Participate in the on-call and emergency coverage schedule
- Participate in all managed care plans
- Oversee care coordination with medical case management and health education
Requirements:
- MD/DO
- Board certified or eligible in Internal or Family Medicine
- Valid NYS medical license
- DEA
- Commitment to serving the underprivileged
